What causes ingrown toenails?

An ingrown toenail, or onychocryptosis, occurs when the corner or side of the nail grows into the adjacent flesh. This can cause redness, swelling, pain and discharge.

There are various reasons why ingrown toenails occur, but commonly they are caused by one or more of the following:

Incorrect cutting technique
Picking rather than cutting toenails
Tight or ill-fitting footwear – socks and stockings included
Trauma
Genetic predisposition
Some medications

How are ingrown toenails treated?

Treatment of your ingrown toenail will depend on its severity. Ingrown toenails rarely improve over time, so the sooner you seek treatment, the better.

Conservative treatment

We have the skill to quickly and carefully remove the piece of nail that has penetrated the skin with little to no discomfort. However, if the site is painful, then a local anaesthetic can be used.

Once the offending piece of nail is removed, a dressing will be applied, and you will then be given instructions on how to care for your toe at home. Typically, you’ll walk out the door, ready to return to shoes and activity within 1-2 days. You will also be educated on how to best prevent your ingrown toenail from reoccurring.

Surgical treatment

For those stubborn, recurrent ingrown toenails, we offer simple, safe, and effective, minimally invasive surgical procedures that provide a permanent fix.

Our surgical procedures are performed in our clinic, under local anaesthetic, and typically take about 1 hour from start to finish. During this procedure, a chemical (Phenol) is applied to the nail root, which prevents any re-growth or recurrence. This is effective in over 95% of cases.

Once complete, you will walk out of the clinic with your toe dressed and instructions on how to care for your toe during the healing process, which is typically 3-4 weeks. During this time, we will ask you to refrain from impact activity (e.g. running and kicking balls) and swimming/getting the toe wet.

Will my private health insurance cover toenail surgery?

This depends on your individual insurance policy. If you have extras containing podiatry cover, it is likely a portion of the fee will be covered.

What causes ingrown toenails?

Fungal laser treatment

The Lunula cold laser is used to eliminate fungal infections and treat other specific nail and skin diseases. It produces impressive results when compared to alternative treatments such as oral medications, topical treatments, thermal laser, surgery, or home remedies.

The Lunula has two laser diodes (beams) which rotate. The beams deliver light to the cells in the form of energy. The cells have photoreceptors which absorb this energy and convert it into chemicals which are occur naturally in the body. The two chemicals that are released by the cells are hydrogen peroxide, which is a natural anti-septic, which attacks the fungal elements present. The other chemical produced is nitric oxide, which is a vasodilator (opens blood vessels) thus increasing blood supply and oxygen to the area. For those stubborn, recurrent ingrown toenails, we offer simple, safe, and effective, minimally invasive surgical procedures that provide a permanent fix.

Our surgical procedures are performed in our Balgowlah clinic, under local anaesthetic, and typically take about 1 hour from start to finish. During this procedure, a chemical (Phenol) is applied to the nail root, which prevents any re-growth or recurrence. This is effective in over 95% of cases.

Foot Care During Pregnancy

As we know, during pregnancy the body undergoes considerable changes. What many people may not realise is just how much pregnancy can affect the feet. Carrying the extra weight of the baby will invariably place more force through the feet which can lead to foot and leg pain. The feet also tend to swell and they can increase in length and width which can make comfortable shoes difficult to find. The swelling can also make ingrown nails more common. And of course, it can be a whole lot harder to actually reach down to care for your feet!

Most foot pain associated with pregnancy, however is easily treated and our podiatrists can help your feet stay comfortable during your pregnancy.

Pregnancy and foot pain

Many women suffer from foot pain during pregnancy for a variety of different reasons. Of course, carrying the extra weight of the baby will place more force through the knees and the feet. This can cause different structures within the foot to be overused and tired. Conditions such as plantar fasciitis, which is an overuse injury of the plantar fascia or the ‘arch’ of the foot is particularly common during pregnancy.

A pregnant belly will also change the centre of gravity within the body, this can change the way that you stand and walk and transition from a ‘sit to stand’ position. These changes require your feet to work in a different and somewhat awkward way which can put a different type of stress and strain through the feet.

As podiatrists we have seen pregnant women who complain of pain at the front of the foot (forefoot pain). A study from Brazil in 2011 examined the plantar pressure distribution in standing and walking, in pregnant women throughout pregnancy. 1

In this study they found during gait, the most prominent changes throughout pregnancy were the increase in contact area and contact time at the midfoot and forefoot. The authors believe this could help maintain stability of pregnant women during gait. A fascinating finding was the increase of contact area at the lateral aspect of the rearfoot, which may be due to a compensatory mechanism resulting from the increased base of gait at the hip joint with the progression of the trimesters.

These results help explain the forefoot pain that pregnant women experience especially in their third trimester. At Myfeet Podiatry we can manufacture orthotics to help reduce pressure and force in this area which help to reduce overuse injuries in this specific patient population.

When pregnant, the extra weight can cause heel pain especially when getting out of bed. Heel pain is not normal and can be avoided. The most common cause of heel pain is plantar fasciitis.

The ligament that attaches from the heel to the toes is called the plantar fascia. The treatment for heel pain is best achieved by supporting the arch. The best way of doing this is using custom orthotics and wearing supportive shoes. When not wearing shoes it is better to wear thong, sandals or slippers that offer more support.

Pregnancy and footwear

Hormonal changes also cause the ligaments and to increase in elasticity and ‘stretch’. The release of a pregnancy hormone called ‘relaxin’ can loosen the joints in the feet which can actually cause your feet to flatten and to essentially become longer. Some women may find that they need to go up a shoe size because of these changes.

Wearing comfortable shoes of the correct style and size can have a huge effect on how your feet and legs feel. It will be important to find shoes that are the correct fit, have ample cushioning as well as appropriate arch support.

Custom foot orthotics are a fantastic way to support your feet and improve your comfort. Your podiatrist is able to design of pair of innersoles customised to your foot shape and your needs. Pregnancy and Feet Swelling

Swelling in the feet is also part of most pregnancies. Blood vessels dilate to support the extra blood required for the baby. This can also cause foot and leg pain as well as fatigue. We are often asked if compression stockings are recommended during pregnancy. A review in 2015 suggested that there is not enough evidence to suggest that compression stockings have any advantages in reducing symptoms caused by swelling in the feet and legs for pregnant women. In this review there was moderate quality evidence to suggest that a drug called rutoside seemed to be effective in reducing symptoms, but unfortunately this study was too small to say this with real confidence. 2 Other non-pharmacological treatments for swelling in the feet and legs during pregnancy include, rest by elevating the feet and legs when possible, reflexology, water immersion and foot massage.

I can’t reach my feet during pregnancy – can a podiatrist help me?

We certainly can! If you unable to reach your feet to manage your own nails, we are here to help. In fact, we would prefer you come to us for your foot care during your pregnancy rather than attempting to do so yourself! We provide treatment for corns, callus, dry heels, nails etc. All the podiatrists at Myfeet Podiatry are highly trained in providing this type of foot care. This means that there will be no cuts or injuries to the foot which could potentially lead to an infection.

The additional force placed on your feet can cause corns or calluses to develop. Women are also more likely to suffer from cracks in the heels from a combination of the additional weight, hormonal changes affecting the skin and gait changes.

Pregnancy and ingrown toenails

Ingrown toenails are an unwanted and tricky condition to deal with at the best of times and they are the last thing you’ll want to deal with when pregnant! If an ingrown toenail develops during pregnancy, then you’ll of course want to go the extra mile to make sure this is resolved in a safe way for both you and your baby.

Swollen feet can make your shoes fit tighter and squeeze more against the toes. This pressure can lead to the development of an ingrown toenail.

During pregnancy you may also notice that your nails are growing faster than before and that reaching down to your toes to cut them correctly may be more difficult. Poor nail cutting during pregnancy may also lead to the development of ingrown nails. Occasionally the nails may also become more brittle and they may be more prone to separating and lifting from the nail bed.

At Myfeet Podiatry any course of action to treat your ingrown toenail while pregnant will be carefully considered.

If we are able to remove the ingrown aspect of the nail conservatively without surgery then this will always be preferred during pregnancy. This involves the podiatrist carefully cutting away the nail from the side of the toe with specialised tools. This is able to fix the vast majority of ingrown toenail cases.

However, if the ingrown nail is unable to be managed using this method then surgical removal may be required. The use of local anaesthetic to numb the area such as lignocaine is known to be safe during pregnancy.

Surgical treatment of an ingrown toenail during pregnancy may vary slightly in that the podiatrist will likely not use a chemical to cauterise the nail bed so that the nail wont grown back.

This is a precaution often taken so that the surgical site can heal quicker and reduce the change of infection following surgery. Not cauterising means that there is a possibility that the nail may grow back ingrown in the future. However, we usually advise that if the nail does grow back ingrown the surgical procedure can be repeated with the use of chemical, for a permanent solution when the patient is no longer pregnant.

How to Know if Your Child Needs Orthotics?

Orthotics – it might be a hot health topic amongst the parents at the school gate. So what are they and why would your child need them? Here’s your complete guide on what to look for and what you should know.

What are orthotics?

Orthotics are an externally applied device that are inserted into shoes to correct faulty foot mechanics. At Myfeet Podiatry we offer different types of orthoses to suit different circumstances.

Why do children need orthotics?

Foot health in children is often ignored and can lead to serious problems – not only in the feet but also in the legs and back. It’s imperative to take note of how your child’s foot is forming and developing as they age. Foot orthotics can help correct any emerging issues that your child might be suffering from.

What are the early signs that your child might need orthotics?

If you’ve noticed any of the following, your child should be assessed by a podiatrist in case they need orthotics:

They have a hard time keeping up with their classmates
Their feet turn inwards
Their feet turn outwards
Their feet are flat (low or no arches)
They have high arches

What are the specific symptoms of foot problems that would require the use of orthotics?

Listen out if your child complains about the following, as this means it’s time to get them assessed by a podiatrist:

Pain in the feet, ankles, knees, legs or back
Pain while walking or running
If your child constantly wants to be carried more than other children in the same age group
If your child tells you that they do not wish to participate in sports

What kind of non-verbal cues should you look for?

Sometimes your kids are tough and won’t be so verbal about their niggling issues. In this instance, keep a look out for:

If they favour one leg over another while walking
If they limp or drag their feet while walking
If they keep tripping over, are slightly clumsy or have a lack of coordination
If they have any stiffness in the foot and ankle
If they have a sudden change in the way that they walk

Also, when helping them to change their shoes, look out for:

Abnormally shaped toes
Ingrown toenails
Bunions (a bony bump that forms on the joint at the base of your big toe) or other foot deformities such as bone spurs (a bony lump that develops along bone edges)

If you have a family history of foot problems, is your child at risk?

Your child will be more likely to suffer from the same problems if there is a family history. Unfortunately, foot problems such as flat feet can be hereditary and are likely to be passed down. In this instance, if you do know of these problems in your family, it’s best to have your child assessed for the same issues.

How old does a child have to be to wear orthotics?

At 12 months, a child may be eligible to wear orthotics. However, this is a rare occurrence. The biggest milestone to really keep an eye on is once they turn two-years-old. If your child isn’t walking by then, then a podiatrist should assess them.

Are there any other specific age milestones that you should look out for?

As a minimum, it’s always good to do a check in on your child’s foot health at these age milestones:

Age three (this is when children develop a more adult “heel-to-toe” gait)
Age five (when starting school)
Before beginning organised sports

How long does it take to get orthotics fitted?

As mentioned earlier, there are many different types of orthotics available including off-the-shelf, mid-range and custom made. Once the podiatrist has performed an assessment and discussed the most suitable option then the orthotics can be fitted in 1 to 2 weeks. It is recommended that your child returns one month after wearing them for a follow-up assessment.

Diabetes is a complex disease that can affect many parts of your body, including your feet. In relation to the feet, diabetes can cause two main problems:

Loss of sensation (nerve damage or peripheral neuropathy)
Restricted circulation and blood flow (peripheral vascular disease)

This means that people with diabetes may not feel (or ‘sense’) the symptoms of foot problems, and foot injuries or infections may take longer to heal due to reduced blood flow.

If you have diabetes, even minor or ordinary foot problems can lead to serious complications. For this reason, it is recommended that you see a podiatrist at least annually, to help detect any changes early before they become a problems. You should arrange to see your podiatrist immediately if something on your foot is not healing or if you find a red swollen area on your foot.

Self care of your feet is particularly important for people with diabetes. If you have diabetes, it is recommended that you inspect your feet every day, looking at each foot for signs of injury including blisters, bruises, cracked or broken skin, cold or hot areas, corns and calluses, and discolouration.

Ingrown toenail surgery is a procedure that, in the majority of cases, provides a permanent solution to ingrown toenails that have become a recurrent problem. At Entire Podiatry, ingrown toenail surgery is performed by qualified and experienced podiatrists in each of our fully equipped clinics.

The ingrown toenail surgical procedure involves three main steps:

Applying a local anesthetic to both sides of the affected toe, using a needle (this local anaesthetic will numb the toe for several hours). Before injecting the local anaesthetic, we will apply a topical anaesthetic skin spray to numb the area and reduce any pain from the injection.

Removing a segment from the side of the nail that has become ingrown.

Sterilising the nail bed (matrix) at the base of the removed section (so that the nail that has been removed doesn’t grown back).

The procedure takes about one hour to perform.

What happens after ingrown toenail surgery?

After ingrown toenail surgery the affected toe will continue to be numb for a few hours, after which you may notice some pain and discomfort. Over-the-counter pain medication will assist with pain management as required.

Ingrown toenail surgery doesn’t require any stitches, however it is necessary to keep the dressing clean and dry for a few days following the procedure until you return for your review appointment. Your podiatrist will provide a series of post-operative instructions to assist with healing and recovery following the procedure.

It will generally take 4 to 6 weeks for full healing after ingrown toenail surgery, however most people enjoy significant pain reduction very soon after the procedure.

What are Custom Insoles or Orthotics?

What is an Orthotic?

An orthotic has many descriptions:

An external mechanical restraining device
A realigning device
A device that restores the natural shock-absorbing qualities of the foot.

The objective of orthotics is to change the ground on which a patient’s walks to help fix their condition by correcting gait and posture.

Like prescription glasses, orthotics are similar – they act as the corrective medium between the ground and the feet, just as the lenses in glasses are designed to correct specific types of vision. Also like glasses, it is crucial that orthotics are worn every day – they are not short-term fix. We believe that orthotics work by correcting the foundation of the body.

While there are hundreds of different innersoles and orthotics available, they can usually be classified into one of three groups:

Off the shelf devices

What are the treatment options and how effective are they?

It is estimated that 10% of Europeans have an active fungal toenail infection, making it one of the more common condition we see in our foot clinics. Fungal toenail infection constitutes approximately 50% of all toenail conditions. Shoes should generally only be worn on the outside, if the inside is worn it may be a sign of foot issues.

The shoe must be at 90 degrees to the supporting surface and not be tilted in either direction. If it is tilted, then the shoe must be replaced.

4 things to look for in new shoes:

1. Shoes with a breathable top help to reduce sweating and infection in the toes, such as those made of leather.

2. Opt for laces or velcro to ensure that the shoe closes properly and the foot is fully supported

3. Ensure that there's enough room around the toe box for the toes to moe and breathe

What is a bunion?

Bunions are a deviation of the big toe where the big toe joint shifts toward the middle of the foot. This can be accompanied by symptoms including pain and a bony growth that turns into a large lump.

Research shows that bunions are extremely prevalent in Ireland, affecting ⅓ of the population. Over the age of 60, it’s suggested to be as high as 70%. When left untreated bunions can also be linked to other risks and complications.

Bunions are caused by a number of factors including a family history of bunions, footwear choices and the way you stand and walk, however misalignment of the foot is often the underlying cause.

How is a bunion treated?

You may have been told that bunion surgery is the only treatment option available however, there is an non surgical treatment for bunions. We have worked treating bunion without surgery for over 20 years.

Conservative management, such as foot mobilisation, is most effective when treatment of the bunion begins as early as possible, even if the sufferer feels no pain. As a result, often by the time a bunion starts to be painful, significant irreversible damage has already occurred.

There are a wide variety of treatments for Achillles Tendonitis and below are what we offer in this clinic.

Strapping can produive support and comfort to the Achilles tendon for short term relief. Your podiatrist may apply some for a few weeks while your heel is healing
A Home Exercise Program would be prescribed to help build strength and flexibility in selected muscle groups. Examples may include strengthening your calf muscles or adding flexibility to your hamstrings. Your podiatrist will assess these areas to see what exactly you need to focus on.
Ischemic Compression and Dry Needling. These forms of manual therapy can help encourage the healing process along. However many people rely on these alone which would not be enough. It is important to take on all advice from your podiatrist to get the best results

Nail Surgery For ingrown toenail Dublin

Partial Nail Avulsion

If you have a persistent, infected or recurrent ingrown toenail, or a fungal nail, then your specialist may suggest having the nail partially or fully removed. The procedure can also be carried out with the use of a chemical called phenol, this is applied to the nail bed to stop the nail from growing back, this is typically applied when you have a recurrent issue. This procedure is usually carried out as a minor outpatient procedure under local anaesthesia, you can leave directly following rather than being admitted into a hospital. Your foot specialist will start by cleaning the toe and injecting local anaesthesia to either side of the base of the toe. Once numb, the section of the nail or whole nail will be cut down to the base of the nail. The nail will be removed and hypergranulation tissue (scab under the nail) will be removed. It’s suggested that you take a cab home and rest for the remainder of the day, you can return to non manual work the following day. For the first 3 weeks you will need to re-dress and salt water bathe the toe daily. At two weeks postoperative you can continue with high impact activities.

Chemical Phenolisation of the Nail bed

If you have had a recurrent ingrown toenail, which has previously been partially or fully moved, then your specialist may advise on excision of the nail bed. This will remove the root that contains the cells where the nail grows, this will stop the nail from growing back. The procedure may be carried out under local anaesthesia or sedation, you will typically be admitted into hospital for half a day. Your surgeon will excise the corner of the base of the nail and remove the root, alongside the affected section of the nail that is causing you pain. This is then closed with stitches. Following the procedure you should rest at home for 3 – 5 days. You should avoid high impact activities for the first 3 – 4 weeks.

Achilles tendinopathy refers to a range of conditions that affect the Achilles Tendon. There are two main types: insertional and non-insertional Achilles Tendinopathy. Insertional tendinopathy affects the area where the tension attaches to the heel bone, while non-insertional tendinopathy occurs in the middle portion of the tendon. Both types can cause pain, swelling and stiffness.

It's important to note that both types of Achilles tendinopathy can be managed through various treatment approaches, such as rest, physical therapy, stretching exercises, orthotics and, in some cases, surgery. Ingrown toenail Removal

An ingrown toenail can be very problematic with pain limiting your activities of daily living with further complications of recurrent infections requiring repeated antibiotic courses.

Our treatment at the Priory Foot Clinic of an ingrown toenail is minimally invasive. We do so by removing the ingrown portion of the nail and using phenol to stop that in-turned portion growing back again. In comparison to the traditional Hospital based open surgery which will have you off your feet for a few weeks while the wounds heals via sutures (that also need to be removed at a later stage).

The wedge excision and phenolisation method we use will have you back on your feet after the procedure with minimal after care as outline below. Active sporting people are usually back to sport with 2 to 3 weeks.

Causes of an ingrown toenail

The most common cause is wearing shoes that crowd your toenails, over time the margin of the toenail turns in on itself. Other causes include; toenails cut too short or not straight across, Injury to the toenail or just having unusual curvature in the toenails.

Treatment options.

Early recognition will allow you modify your foot wear or how you have being cutting them. Lifting a corner of the nail by inserting a wisp of cotton wool over months may also be enough. Failing this you might need to have the ingrown section treated by an operation.

Wedge excision and nail matrix phenolisation

With the application of local anesthetic to the big toe (called a sensory ring block) the sensory nerves supplying the toe are numbed so from that point on you should not experience pain.

We are conscious of the fear that patients can have of needles and we therefore use the smallest diameter one (needle we also use in anti-wrinkle injections to the face) and most people do not experience much in the way of discomfort. We allow the local to take effect over 15-20 minutes and will last up to 3 hours.

As detailed above the section of the ingrown nail is removed and then a small cotton swab containing phenol is applied to the nail matrix. The phenol chemical destroys that section of the nail matrix not allowing the ingrown section to regrow. The resulting nail width will be smaller, cosmetically we keep this to a minimum and will be discussed with you at the preoperative consultation .

Your foot wound is then dressed with a small anti-bacterial wick with an overlay of a compression dressing which will allow your foot to go into a wide fitting shoe or an open sandal.

The procedure takes about 60 minutes.

After care postoperatively

We at the Priory Foot Clinic would ask you to elevate your foot above your heart level on a couch (supported by pillows) for the remained of the day. This in addition to the compression dressing fitted greatly reduces chance of swelling and a quick return to your normal activities.

The dressing we fit is worn for 2 days. After this you will need to bath your foot twice a day in a chlorhexidine (eg salvon) foot baths or if the skin is quite soft salt baths (eg. epsom salts) for the next 2 to 3 days and cover with a breathable non stick bandage (eg. mepore) if wearing a shoe.

Wearing open top shoes such as sandal promotes hardening of the skin. If you have any concerns please contact the clinic for a review appointment.

What do I need to do now

You may have been told by a healthcare profession that you need to have the ingrown toenail removed as it has failed conservative measures and they can refer you onto our service or you can self refer.

We welcome you for a preoperative consult if you are unsure as to the need of the procedure or if you feel the toe might be infected. It is important that any infection is treated as this reduces the effectiveness of the local anesthetic and slow the healing process post-op.

After Care

Chilblains usually develop several hours after exposure to the cold. They typically cause a burning and itching sensation in the affected areas, which can become more intense if you go into a warm room.

The affected skin may also swell and turn red or dark blue.

In severe cases, the surface of the skin may break and sores or blisters can develop.

It’s important not to scratch the skin as it can break easily and become infected.

Do not keep your feet warm by:

keeping a hot water bottle beside them in bed
Putting them close to the fire

Keep your feet warm by:

Wearing thick waterproof footwear for example boots. Boots also provide better grip for when walking in the icy conditions.
Wearing thick socks or multiple layers of thin socks, layers trap heat!
If wearing tights to school or work wear a thin pair of socks underneath!
If you work outdoors try adding a thermal insole for extra heat! (may not be recommended for people with diabetes)

How to diagnose Chilblains:

Look for dark areas around the distal end of your toes and feet. They should be cold to touch and itchy.

Paediatric Intoeing - What is it??

Paediatric Intoeing

Signs of intoeing:

Feet turn inward when walking, resulting in a ‘pigeon toe’ appearance. Both or only one foot may turn inwards.
Difficulty with coordination during activities -prone to tripping and falling when running.
Sitting in the ‘W-position’.

Causes of intoeing:

Metatarsus adductus - the foot has a curved shape and the big toe deviates away from the lesser digits. This is common in infants and usually resolves by 2-3 years of age.
Internal Tibial Rotation - The knees are front facing but the feet turn inwards. This usually resolves naturally by 6 to 8 years of age.
Femoral Anteversion - Most common cause of intoeing, where the thigh bone is rotated internally, causing the legs to turn inwards giving a ‘knock knee’ appearance. It usually resolves by 6-8 years, but is still considered normal into the early teenage years.

In our clinic:

The podiatrist will carry out a thorough assessment to determine the cause of the intoeing, including a gait assessment, testing joint range of motion of the hips, knees and ankles and examining for any muscle imbalances.

If the intoeing is not resolving on its own, we can help encourage resolution by:

Prescribing exercises to strengthen the outer hip muscles
Stretching exercises for the inner hip muscles
Discourage sitting positions such as the W position
Prescription of paediatric insoles to assist with support and stability when walking.
Referral to orthopaedics if required.

Buying school shoes for your child can be tough. With school dress codes, budgets and growth spurts all thrown into the mix – there is a lot to consider. Before you start your school shoe shopping, check out this interactive guide to help make finding the perfect pair of school shoes for your child, easy.

Before you shop

It is important to remember that shoes are the most important part of your child’s school wardrobe, as they will be wearing them for 35 to 40 hours per week. Because of this, it should be a top priority to find shoes that are comfortable, supportive and – above all – the perfect fit. An ill-fitting shoe can lead to long-term problems with their feet, legs and spinal health, so it is best to get it right from the start.

Before heading to the shops, you will need to answer a few questions:

What style of shoe will you be buying? Will you look for a more traditional leather school shoe or a running/training shoe? Perhaps both!
How active is your child?
How much money are you willing to spend?

To answer these, there are a few things to consider.

Dress Code

First, find out if the school has any regulations about what kind of shoes your child can wear. Some schools will enforce a stricter dress code and only allow full black leather shoes, whilst others will allow kids to wear nearly any type of enclosed shoe.

Running Shoes

Lots of children wear running shoes as their main school shoe. However, it is important to understand that running shoes are made for – as the name suggests – running. The tread is only designed to go in one direction and as soon as your child starts changing direction on the playground (e.g. stopping, starting and making other sudden movements) the sole will wear away very quickly. This means that the shoe will need to be replaced more regularly, well before your child grows out of the shoe.

Activity Level

Another factor to consider is how active your child is. If they are very active and don’t have dress-code restrictions, you may want to opt for a cross-trainer. These shoes usually have better ankle support, a deeper tread and will last much longer than a standard running shoe. If they are active and also have a strict dress code, shop around for a leather shoe with a sportier and more robust tread.

Budget for Growth

Depending on how old your child is, you will need to think ahead about how many pairs of shoes you will need to buy this year. Children’s feet grow in spurts; expect these shoe size changes:

The 2 to 3-year-old: Grows one-half shoe size every four months
The 3 to 4-year-old: Grows one-half shoe size every four to six months
The 4 to 8-year-old: Outgrows shoes every six months
The 9 to 12-year-old: Outgrows shoes every six months to 12 months

What features and types of shoes to avoid

As well as understanding what positive attributes to look for when shopping, there are also some features and styles of shoes that should be avoided. It is recommended to:

Avoid buying shoes that are too heavy, which will be harder to walk in and could cause pain.
Avoid buying overly stiff, thick-soled school shoes. These kinds of shoes reduce sensory information getting through to the brain.
Avoid buying rubber clogs (e.g.: crocs) as school shoes. They are not suitable for all-day use. They offer no heel stability and as toes tend to grip they can lead to tendonitis, worsening of toe deformities, nail problems, corns and calluses.
Avoid buying ballet flats as they have no cushioning or support, which can lead to heel pain, plantar fasciitis and pain under the ball of the foot.

Wearing in shoes

Once you have bought your shoes, it is important to allow some time for your child to wear them in before school starts. How do you go about it? Here’s our advice:

It’s best for children to wear in school shoes gradually, one to two weeks before they have to wear them for full school days.
School shoes and socks can be worn for short periods at home, starting with one hour per day, and increasing by an hour each day.
Ideally, your child should be able to tolerate wearing their school shoes and socks at home for five consecutive hours before starting to wear them for school.

If your child is experiencing any pain, redness or skin irritation when wearing their new school shoes, it is important to get it checked out by a podiatrist. A poorly fitted shoe can lead to more serious problems later down the track.

PLANTAR FASCIITIS

WHAT IS PLANTAR FASCIITIS?

Plantar fasciitis is a painful inflammatory heel condition affecting the large ligament-type structure in the base of the foot that can cause arch pain and heel pain. The plantar fascia can become thick and inflamed due to the irritation at its attachment, at the base of the heel bone. In some patients, small tears can develop and these people suffer from acute pain that can be excruciating. Sometimes, on x-ray, heel spurs can be seen, but these are not usually the cause of the patient’s heel pain.

It should be noted that “heel spurs” is a misleading diagnosis that has been used by podiatrists and doctors for many years. More and more practitioners today are open to the fact that heel spurs are not the problem and that these bone spurs don’t cause pain. Instead, the source of pain is the inflammation within the plantar fascia surrounding the spur itself.

WHAT DOES IT FEEL LIKE?

Patients with plantar fasciitis usually feel pain under the ball of the heel and it can represent a stone bruise sensation. Arch pain is less common but is also a key symptom of the condition. The heel pain is common first thing in the morning when climbing out of bed and after periods of rest, such as driving or being seated. The pain in the heel can be erratic and often, there are pain-free periods when the person feels like they are healing. If there is a very sudden onset of pain, as opposed to a gradual onset, this may be due to tearing of the plantar fascia.

CORRECT DIAGNOSIS

It is important that the podiatrist differentiates between regular plantar fasciitis, and plantar fascia tearing as the treatment is different. Your podiatrist will also be able to exclude the possibility of a stress fracture, which sometimes leads to similar symptoms. Ultrasound imaging may be necessary to accurately diagnose the cause of the patient’s heel pain.

THE STAGES OF PLANTAR FASCIITIS

INITIAL STAGES OF PLANTAR FASCIITIS

Initially, these heel pain patients may feel a dull ache in the base of the heel. There may be an awareness of a problem in the afternoons, or after they have been weight-bearing for long periods of time. It is possible that they feel some pulling in the plantar fascia, which they describe as a tightness in the arch or the sole of the foot.

Early treatment or intervention at this stage is crucial and may inhibit the development of a more chronic heel pain condition. Unfortunately, many patients ignore the early symptoms, presuming or hoping that the tightness or the dull ache in the heel will settle down and dissipate. In some people, this is the case, but others can develop severe heel pain and/or chronic P.F.

As sports podiatrists, the typical symptoms that we hear in these early stages, are the feeling of a pebble in the shoe or a stone bruise sensation. Patients with mild P.F sometimes inform us that they felt like they had stepped on a small pebble or stone.

In these early stages that we describe above, the condition does not always induce heel pain throughout the daytime. The symptoms may only be present in the morning when getting out of bed and can be very sporadic. Some patients are asymptomatic for a day or two only to find the pain returning.

In the early stages, there can also be some heel pain in the morning when the foot hits the floor, but this is usually mild and is often little more than a feeling of tightness.

CHRONIC PLANTAR FASCIITIS

When the condition is fully developed, the symptoms of plantar fasciitis heel pain are more acute. The condition will become chronic if the strain on the plantar fascia goes on for long enough and remains untreated.

The pain in the heel is more consistent and patients will usually report pain, not just stiffness, each morning when hopping out of bed. Most patients hobble for at least 5 minutes after getting out of bed due to extreme plantar fasciitis heel pain.

The pain in the heel usually subsides within 15 minutes and patients are usually more comfortable after taking a hot shower. Walking throughout the day isn’t always a problem as the inflammatory change in the plantar fascia is less consistent and the heel pain isn’t always present.

However, mid-afternoon, after several thousand steps the foot fatigues, and the heel pain returns. Patients will sometimes report pain when getting out of their car after a drive home from work. They hobble again for the first few minutes and find some relief after walking a short distance.

Podiatrists are often informed by their patients that they also limp when they stand up after being seated for dinner in the evenings or after sitting on the couch watching television. An ultrasound report will often show thickening of the plantar fascia, due to inflammatory change.

SEVERE PLANTAR FASCIITIS

If the condition comes on very quickly, during a one-off event or during a particular day/activity then the pain can feel severe/acute. Alternatively, chronic plantar fasciitis left untreated can lead to the same state.

ACUTE INFLAMMATION

In these cases, the strain on the plantar fascia has been so great and so repetitive that acute inflammation develops as does extreme pain. Patients with severe P.F will usually describe to the sports podiatrist an extreme level of pain that is present for most of the day. These patients will experience throbbing when sitting or lying down and are starting to feel the psychological effects of having a condition that is both chronic and acute.

Often described as “excruciating” or “relentless”, heel pain plantar fasciitis patients experience symptoms that affect their daily lives, as they stop exercising and are often struggling to walk during day-to-day movements.

Some patients with severe P.F have developed deep surface or laminar tears in the fascia. As sports podiatrists, we arrange ultrasound scans, to help confirm or exclude torn fascia. X-rays are less helpful in the case of this soft tissue injury.

WHO SUFFERS FROM PLANTAR FASCIITIS HEEL PAIN?

ADULTS

Plantar fasciitis can affect men and women equally, but there is a trend in terms of the age of the patients that we treat. Fewer people younger than 30 years of age develop the condition, and the majority of patients are over 60. In between those ages, there is a large percentage of male and female patients in their 40’s and 50’s.

Sedentary people who haven’t exercised for some time, who enter a get fit health program of sorts, and those who join a boot camp, will often develop pain in the heel or Achilles tendon, namely plantar fasciitis, Achilles Tendonitis or other rearfoot issues, is crucial in these cases to prevent further complications.

EXPERIENCED PODIATRISTS

CAUSES OF PLANTAR FASCIITIS

MULTIFACTORIAL CAUSES

There is no single cause of plantar fasciitis. The cause is multifactorial, as is the plantar fasciitis treatment. Ultimately, there is an increased load on the plantar fascia which causes it to become pathological. Tight calf muscles and soft shoes are a big problem, as are flat feet and an increase in body weight. Biomechanical issues leading to poor foot function can also trigger the condition. Patients who suddenly increase the amount or frequency of sporting activity/exercise are also at risk.

We often hear patients saying, “I was looking to get fit after Christmas and was training several times a week, and then my heel became extremely painful…”

Paediatric flat feet, also known as pes planus, is a condition where
the arch of the foot is lowered.
There are two main types:
Flexible flat foot: due to tissues in the feet being loose or
flexible. Very young children have added soft tissue and flexibility,
which can make feet look flatter than they are. A flexible flat foot
will have an arch when the child is sitting, when their big toe is
bent backwards or if they stand on their tiptoes. An arch will develop
around the age of 6.
Pathological flat foot: the child's feet are flat due to disease or a
condition. Possible causes include a neuromuscular disease (cerebral
palsy, spina bifida), a rigid flat foot, genetic disorders (Down
syndrome) or injury.

Treatment:
If the foot is flexible with no symptoms, no treatment is required. It
is likely the child will grow out of it. If pain is present,
paediatric orthotics, exercises and footwear advice should be given.

Plantar Fasciitis vs Heel Spur

Heel pain is very common amongst the general population. The two main causes of heel pain is plantar fasciitis or a heel spur. They can be similar in presentation, however it is important to distinguish between the two, to ensure proper treatment of the area.

Plantar Fasciitis - inflammation of the ligament along the sole of the foot

Symptoms: morning pain or pain after rest, gradually gotten worse over time, centre to medial aspect of heel

Cause: overuse, long periods of standing, weight gain, inappropriate footwear

Treatment: exercise regime, ice, footwear, orthotics

Heel Spur - bony growth to the base of the heel

Symptoms: severe pain to direct point in heel

Cause: repeated stress on heel, due to excessive activity, overuse, inappropriate footwear

Treatment: padding around heel, ice, footwear, orthotics

Varicose Veins and the feet

Varicose Veins are swollen and enlarged veins that usually occur on
the legs and feet. They may be blue or dark purple in colour, and are
often lumpy or twisted in appearance. They are caused by blood pooling
in the veins, causing them to bulge.

Varicose veins can cause pain, swelling and discomfort in the feet. A
feeling of aching/heaviness is common. Burning, throbbing, cramping or
swelling in the lower limbs is also reported. Pain is usually worse
after sitting or standing for a long period of time. Varicose veins
can increase the risk of blood clots and ulcerations.

Risk factors for developing varicose veins include:
1) age - wear and tear on the veins throughout life
2) sex - women are more likely, due to hormonal changes and/or
contraception pill
3) pregnancy - increased blood volume during pregnancy increases risk
of varicose veins
4) obesity - increased weight increases pressure to veins

Treatment options for varicose veins in the feet include compression
stockings, lifestyle changes and possibly a need for surgical
intervention.

Athletes Foot- What is it? What do I do?

What is Athlete's Foot?

Athlete’s foot, also known as tinea pedis, is a fungal infection of the feet, that can affect the skin and nails.

Causes?

Direct contact with someone with active tinea pedis
Thrives in warm, moist, damp conditions increasing the risk of infection in shower rooms, bathroom sand swimming pools

Risk Factors:

Feet prone to perspiring
Sharing socks, shoes or towel with someone with an active infection
Keeping feet wet for long periods of time
Walking barefoot in public areas - eg. swimming pool, gym changing rooms

Symptoms:

Fissures or cracks in the skin between the toes
Redness and itching between the toes
Dry blistering on the feet
Discoloured, brittle and thickened toenails

Treatment:

Over the counter topical antifungal powders and creams - eg. Daktarin
In severe cases, a GP may prescribe oral antifungal medication

Age: 56

Presenting Complaint:

Pain in the right forefoot. Patient reported pain is at its worst when pressure is applied between her 2/3 ^rd ID space, particularly when she is driving and her foot is up on the gas pedal. Patient reported she has become more active recently and noticed the pain soon after activity. Pain was described as tingling/burning and is worse with friction. The patient is awaiting an MRI to investigate the area.

Treatment:

Hands-on assessment alongside thorough history take was conducted. A positive Mulders click test was found and thus a mortons neuroma was suspected. Differentials such as plantar plate/bursitis/fracture were also investigated. However, mortons neuroma was most likely after assessment, particularly with the presentation of HAV bilaterally.

The patient was educated on the role of footwear and the importance of wide-fitting toe and deep toe box. Short-term management included an insertion of a metatarsal dome to splay the digits and alleviate the stress and friction.

Long-term patient will be reviewed following results of MRI scan and options for long-term orthosis will be considered.

Age: 47

Presenting Complaint:

The patient attended our foot clinic with pain in the left medial calcaneus. The patient had an x-ray completed of the left heel which presented with a spur. Heel spurs occur due to chronic overloading and stress. The patient also presented with chronic plantar fasciitis; this is a common occurrence alongside heel spurs. The patient reports she has ceased sports and would like to restart playing tennis as soon as possible.

Treatment:

A full biomechanical assessment was carried out with weight-bearing and non-weightearing assessments as well as gait analysis on the treadmill. A 3D scan was undertaken to cast the foot. A customized orthotic with heel cushioning and 1 ^st metatarsal cut-out to effectively offload stressed tissues was prescribed. This was implemented for the long term. Stretching and strengthening exercises and offloading of the heel short-term with a SCF cutout specific to an area of tenderness on palpation, which drastically improved symptoms upon review

What treatments do we do for heel pain?

There are various types of heel pain and once your podiatrist has diagnosed it and determined the cause of it, he / she may advise the following.

FOOTWEAR - a simple change in footwear may be all you need. Your podiatrist will educate you on the characteristics of a shoe to look for and avoid for your heel pain

EXERCISES - your podiatrist will go through exercises with you. Targeting the various muscle groups needed. It is important to do these exercises as prescribed by your podiatrist.
As a reminder our podiatrist can email you the exercises or give you a printed sheet with instructions.

ICE - your podiatrist may advise you to ice the area.

STRAPPING - Your podiatrist may apply tape for support short term to give the heel time to heal.

ORTHOTICS - if you have had long standing pain, orthotics may be required to improve the biomechanics of the foot to reduce pressure on the heel.

Each case is different, and it may take a combination of a few treatments mentioned to reduce your heel pain.

Firstly, your Podiatrist will take a brief medical history and discuss any relevant previous or ongoing issues you may of had regarding your feet.

The Podiatrist will ask you some questions regarding your heel pain such as;

The Podiatrist at the podiatry clinic south Dublin will assess your footwear looking at wear patterns and ask some questions regarding the shoes you wear the most on a regular basis, if you wear high heels or can you cut back on wearing a certain type of shoe for a short time to allow for healing.

The Podiatrist at the Merrion Foot Clinic will carry out a biomechanical examination which will involve a joint assessment, static assessment (while you are standing), dynamic assessment (while you are walking) and muscle strength testing.

After gathering all the relevant information from your history and assessments the Podiatrist will form a treatment plan for your heel pain according to the cause. This may involve rest, icing the area, footwear advice, strengthening exercises and/or orthotic therapy.

I have Bunions! What should I do?

Firstly, what is a bunion?

A bunion is a deformity of your big toe joint. The fancy term for this is ‘hallux abducto valgus’.

But don’t panic, bunions are very common! Women are 3 times more likely to develop a bunion than men. The incidence among women is estimated to be as high as 50%!

How did I get a bunion?

The jury is out on this one… There is no definite cause for bunions.

However, here is a list of risk factors associated with bunions.

Ok, so what should I do about my bunion?

Bunions do not cause pain for many people. As a general rule, don’t fix problems that are not there.

Fungal Nail Infections

Fungal nail infections generally present in 4 different forms.

This is a fungal nail infection that presents near the nail bed. Proximal subungual onychomycosis is a type of fungal nail infection in which invasion begins from the undersurface of the proximal nail fold and then progresses distally. The nail changes are either diffuse patches or transverse striate patterns. The striate pattern may be associated with superficial onychomycosis .

2. Distal Subungual Onychomycosis

This fungal nail infection presents near the nail’s loose edge. It is characterised by yellow discolouration, thickening and crumbling of the nail. This is the most common fungal nail infection and is closely associated with athl etes’ foot. Distal subungual onychomycosis begins under the leading edge of the nail or along the lateral edges and is characterized by the accumulation of crumbling subungual debris and thickened, discolored nails. In proximal subungual onychomycosis , infection begins at the proximal nail fold with infection of the nail matrix.

3. White Superficial Onychomycosis

This fungal infection presents itself as white plaques on the surface of the nail. White superficial onychomycosis (WSO) is a form of onychomycosis characterized by a superficial localization of fungi on the dorsal surface of the nail plate. It may be easily treated by mechanical debridement of the affected area, followed by the application of topical antifungal agents.

People often confuse this with ‘dryness’ following removal of nail polish

4.Total Dystrophic Onychomycosis

This is an advanced form of fungal nail infection that infects the whole nail. The nail becomes thick and crumbles. Total dystrophic onychomycosis or severe onychomycosis refers to the most advanced form of any of the subtypes. This form of onychomycosis is a later stage of the chronic subungual dermatophyte infection that may take 10 to 15 years to develop.

What is a verruca?

A verruca is a wart that appears on the bottom of your feet. For most
people they do not cause any pain but occasionally if they become large
enough, they can cause discomfort walking. A verruca is caused by the
human papilloma virus (HPV). There are over 100 strains of HPV. Most
HPV strains are benign and do not cause any serious complications.

How did I get my verruca?

Verrucae are transferred from person to person by skin to skin contact or
on warm moist surfaces. A lot of people pick up verrucae in communal
changing areas, showers, or swimming pools. You should always avoid
walking barefoot in these areas!

What should I do if I think I have a verruca?

If you think you have a verruca you should visit your podiatrist where
they can tailor a treatment plan to you and your lifestyle.
